For your first appointment at Aptitude Physical Thearpy, please arrive half an hour before your appointments time to finish the patient registration process. You may print off and fill out the attached documents to bring to your first appointment if you wish.

At your first appointment at Aptitude Physical Therapy, please bring your driver's license or government-issued proof of identity. If we are filing your insurance claim, please bring your health insurance card.
Please note that while the Aptitude PT staff will help with filing claims to some insurance companies, patients are responsible for all charges.
